Client History

View Answered and Unanswered Intake Questions

Objective: Review a client’s intake to see which questions have been answered, which sections are incomplete, and whether any items still require attention.

How to:

  1. Access the Client History
    • Navigate to a client’s chart
    • Click on Client History
    • You’ll be redirected to a list of collapsed sections from the Intake Forms (e.g., General Health, Lifestyle, Current Pregnancy).

  1. Check Intake Section Status
    • Click on any main section to uncollapse it
    • A list of sub-sections appears
    • All items will display “Answered” or “Not answered” labels when no data has been submitted yet

Accept Intake Form Answers

Objective:  To review and accept answers submitted through the Intake Forms to confirm they are accurate and complete.

How to:

  1. Accept Submitted Answers
    • Using a client's chart with answered intake forms, go to the Client History tab
    • Click on a section you previously completed (e.g., General Health)
    • Sub-sections now show answers
    • Click Accept next to any sub-section
    • The Accept button disappears once clicked, confirming the data has been accepted.


Edit Intake Form Answers

Objective: Update or correct previously submitted responses from the Intake Forms. If the client has not completed the intake, the provider can enter responses during the visit.

How to:

  1. Access the Client History
    • Navigate to the client’s chart and go to the Client History tab.
  2. Edit a Section
    • Expand any section containing answers
    • Click the Edit button next to a sub-section
    • A sidebar appears with the current answers pre-filled
    • Make any changes and click Update to save
