Amend Encounter
Amend a Completed Encounter
Objective: You want to update information in a previously signed encounter, while preserving the original record through amendment history.
How to:
- Access the Encounter
- Go to a client's chart and click the Encounters tab.
- You’ll see a list of In Progress and Completed encounters.
- Open a Completed Encounter
- Click on the completed encounter to open it.
- In the top-right corner, click Amend Encounter.
- Submit an Amendment
- The amendment form opens, pre-filled with the original encounter details.
- Under Amendment Info, choose a reason:
- Provider correction of information
- Client requested change
- Other (includes a free-text “Please specify” field)
- Add a description of what has changed.
- (Optional) Check Notify client of changes.
- Click Sign & Lock.
- The updated information is saved and replaces the visible version of the encounter.
View Amendment History
Objective: You want to see past amendments for an encounter and review what changed.
How to:
- Access the Encounter
- Go to the client's Encounters tab.
- Open any completed encounter.
- Open Amendment History
- Scroll to the Amendment History Details section.
- Click View Amendment History.
- Review Past Amendments
- A list of all amendments appears with authors, dates, and summary info.
- Click View Amendment next to any entry.
- The Amendment Details page shows what changed between versions.
Tips:
● All amendments are versioned and preserved for audit. ● Use clear amendment notes to document your reason for the change. ● Notify the client when changes are clinically relevant.
