Amend Encounter

Amend a Completed Encounter

Objective: You want to update information in a previously signed encounter, while preserving the original record through amendment history.

How to:

  1. Access the Encounter      
    1. Go to a client's chart and click the Encounters tab.     
    2. You’ll see a list of In Progress and Completed encounters.
  2. Open a Completed Encounter     
    1. Click on the completed encounter to open it.     
    2. In the top-right corner, click Amend Encounter.
  3. Submit an Amendment     
    1. The amendment form opens, pre-filled with the original encounter details.     
    2. Under Amendment Info, choose a reason:      
      1. Provider correction of information         
      2. Client requested change         
      3. Other (includes a free-text “Please specify” field)     
    3. Add a description of what has changed.     
    4. (Optional) Check Notify client of changes.     
    5. Click Sign & Lock.     
    6. The updated information is saved and replaces the visible version of the encounter.

View Amendment History

Objective: You want to see past amendments for an encounter and review what changed.

How to:

  1. Access the Encounter     
    1. Go to the client's Encounters tab.     
    2. Open any completed encounter.
  2. Open Amendment History      
    1. Scroll to the Amendment History Details section.     
    2. Click View Amendment History.
  3. Review Past Amendments     
    1. A list of all amendments appears with authors, dates, and summary info.     
    2. Click View Amendment next to any entry.     
    3. The Amendment Details page shows what changed between versions.

Tips:

● All amendments are versioned and preserved for audit. ● Use clear amendment notes to document your reason for the change. ● Notify the client when changes are clinically relevant.
